A judge in Hawaii issued an arrest warrant for a 24-year-old woman who missed her court appearance on Wednesday after allegedly faking a COVID-19 vaccination card.

Chloe Mrozak, of Illinois, was arrested in August and charged on suspicion of falsified vaccination documents after she showed an alleged fake document at the Honolulu airport in an effort to avoid Hawaii's mandatory 10-day quarantine. On the vaccine card, Moderna had been misspelled as "Maderna."

Mrozak was released without bail on two misdemeanor charges for violating Hawaii's emergency COVID-19 policies but missed her Wednesday court hearing, held over Zoom, according to The Associated Press.

In response to her absence, Judge Karin Holma issued a $500 warrant for Mrozak's arrest, the AP reported.

Though Hawaii's public defender’s office represented Mrozak at previous hearings, it was unclear to the AP if she hired legal representation for future proceedings. 

“Our office did not receive an application from her for our services,” Public Defender James Tabe told the outlet. “Therefore, we are not representing her.”

The AP said that Mrozak hung up a reporter who called to ask about the allegations on Wednesday.
